  2. Fill out the New Patient Paperwork here to save 15 minutes on your first visit.

Your First Visit
At Shepard Pain and Performance Care, we strive to make your first visit as easy and seamless as possible. During this initial visit you will be given an office tour, will sit one-on-one with Dr. Shepard for a complimentary consultation, receive a thorough and customized examination, as well as enjoy refreshments and other new patient perks.

Dr. Shepard will go over examination results in length with you and may give you an initial A.R.T. treatment.

No Charge for the First Visit
There is no charge for the first visit. This first appointment is an opportunity for Dr. Shepard to examine you and see if you qualify for treatment at our office.

How Long Will the First Visit Take?
A typical first visit will last about an hour. Most of this time is spent with Dr. Shepard. Please fill out our online paperwork before you come in to allow for more time with Dr. Shepard. You can fill out the New Patient paperwork HERE.

What to Wear
Worried about what to wear for your visit? Worry no longer! During your visit, we will provide a t-shirt and shorts for you to change into just in case the clothes you’re wearing aren’t ideal for treatment (like skirts or a sweater, for instance). So don’t worry about rushing home to change before your appointment. Just head right on over!
